2. Current Diagnosis

3. Current condition : Please provide details about your current illness and physical condition

4. Chief complaint (Major problem and concerns): Please provide details about the treatment you received in your country.

5. Patient medical history : Please let me know if you have any other diseases that have been diagnosed, such as high blood pressure, diabetes, hepatitis, allergies, Admission & Surgery history, tuberculosis, etc.

6. Medication list: Please fill in the Medication list and dosage in English.

7. High Risk : Please check if below list applies to the patient ( Emergent Patient / Coma / on Life support / Hospitalized / Chemotherapy )
